Work Permit for a 14 or 15 Year Old Minor
As mandated under State law, work permits can only be issued for jobs in West Virginia. Please follow these four steps in the following order so that your permit can be issued:

1. The minor’s prospective employer shall complete Section A, to include a description of the minor’s intended work activities and any equipment or machines that minor will use.

2. After the prospective employer completes Section A, the minor’s parent or legal guardian shall complete Section B, giving consent for his or her child to work for the prospective employer and shall attach the minor’s certified birth certificate;

3. After Sections A and B are completed, the minor’s school principal or registrar shall complete Section C, certifying that the minor is currently attending school;

4. After Sections A, B, and C are completed, please contact a school counselor, Attendance Director, or Superintendent of Schools to process your work permit. Please bring the minor’s birth certificate or certified copy of the birth certificate.

Age Certificate (16 or 17 year old)

(The certificate is not required under State law, but the law directs an administrative agency to issue the certificate on request from the Employer.) In order to process an Age Certificate, the minor shall furnish a certified birth certificate to a school guidance counselor, Attendance Director, or Superintendent of Schools. Upon receipt of the minor’s certified birth certificate establishing that he or she is 16 years old or older, and a determination that the employer’s business or industry and the minor’s occupation or job are permissible under the Act and this rule, the authorized person shall issue an age certificate to the employer.
